Both Start Server.cmd and PeacockPatcher.exe have to be running before launching the game. Usually the main culprit for saving issues is that the game files are located in a directory that has limited permissions. Which is why the solution is often to run the game with Administrator permissions so that it has elevated privileges to write.

Like you save in the middle of an NPC doing something, and then 10 seconds after you load that save, the game crashes. Happens every time with certain saves.
